I found one!
I completed the Restoring The Faith quest alone, then went to New Garand and picked up Hirge as a companion near the altar. However, I mentioned that now - walking with Hirge when I talk to her I have no option to sent her away! Now she will eternally follow me and I can't team up with anyone else except for mercs, so sad...

Not sure if it matters, but the quest Three Versus Eleven is currently active - may that be an issue?

UPD:
And then I went through a Mysterious Letter path in New Garand which forced the Hirge to finally leave me alone. When I came up the streets, she rejoined back, but this time, thank god, there was an option to sent her away. So everything is back to normal again. Well, I guess the sewer smell I brought on me made her change her mind about my company... :?

Mod Note: Thanks for reporting! Edited thread tittle to better reflect post. Vague tittles aren't that useful so a concise tittle that reflects the post is preferred.

Re: Bugs in 1.054

One more bug. When picking up on of your companions on a street, it reappears close to you and starts following, right? But the thing is that to talk to him/her you still need to return to the position where the companion was just before picked. If you walk a little aside and face the companion, there is no portrait to reference, it is like if it stuck at the old position. This bug disappears once you move to the other location - only then you can refer to your companion by facing him/her. Definitely a bug. And I think it is kinda old one - it was so even before the last update.

This is the first I've heard of the inability to dismiss Hirge.... that could be problematic.

OK, I found the reason. Please complete the Mysterious Letter quest and you'll be able to dismiss her.

I am not sure why exactly I put that limitation a year ago, but at the time I am reticent to remove it... I probably did it for a reason. While I analize it more calmly, the easy solution is simply to complete the letter quest.
