Question about drops/drop rates

Question:
For Items that "normally" drop for certain bosses, do they always drop for them or are they random? I ask because certain weapons/items that have previously dropped by these bosses or found in a chest after beating them don't always show up.

Another thing that's happend to me is this:
I accidentally sold the executioner's axe, re-started from a save before i had acquired it, but it wasn't there anymore.

I think drops vary depending on what time the mob is killed. I can't speak for normal bosses, but I experimented with one of the random rumor spawn dragons. I saved near it and the drops changed every hour of game time. It was consistent too, if I killed the boss at 7 am the drops were always the same.

Drop rate not depends from time. I test it on rumor bosses.
Try to test this steps: save before battle with boss -> kill the boss and take loot -> load save and fight with boss but not win -> die and load your save -> kill the boss and take anotherloot :)

Hour of killing will be the same, but loot different

Talking about chests, I've seen that the same chest can drop random stuff from a kind of table and if you die in the dungeon the loot will be different. My question is: Is there a random difficulty for dungeons? To be clear, some areas have different enemies . I.e. You fight a skeleton archer in the first run, you kill it and take something from the chest, then you die in the dungeon, reload and find a skeletal evoker in that room with a better loot. Is it my impression or you'll find always a better overall loot than the previous run?

No its random. I have found awesome loot, died and then went back and found total garbage.
