Missing unique items

Earlier on this playthrough I couldn't find one of the ruby keys or a weapon that was supposed to be dropped by the boss in the Cursed Abbey quest. Now no matter what I do I can't seem to get the hammer for the Primal Forge quest. I've tried sleeping to reset everything, but the chest that's supposed to have it is still empty. It's pretty discouraging. There's no way to fix this, is there?

Primary Forge.
What your quest log say exactly?
It's possible you need to leave the dungeon, reload and go back into the dungeon to ensure the item was added if there was a map loading error. Also, double check you're looking at the correct chest.

Cursed Abbey
The Abbot does NOT drop a red ruby key. The red ruby key in the area is found behind a secret door on the north wall of the area. See here.

Thank you for the quick response! I thought I was going crazy.
I must've misread the wiki when I was looking for that key? Back when I was playing through the Abbey I ended up having to come back with equipment to boost my Perception to have access to some of the secret doors, so it's possible I missed that.
As for the hammer... I left the cave to explore the Ashen Wastes and go through the Sunken Citadel. When I was ready to unload some of my excess inventory I realized I had the hammer the whole time and somehow didn't see it even though I explored the cave at least three times. I feel a bit sheepish.
Must be time for bed.

What do you mean by that? If you're talking about when I said I had to boost my perception what I meant was that I went to town and grabbed a ring with "detection" on it since I kept going back and forth in front of the wall with the secret door without triggering it.

I just mean cast a scroll. Doesn't matter either way, all roads lead to the same place.
