I string Adaon along to detect and disable traps. However, sometimes, instead of stopping next to the trap to disarm it, he runs straight into it, taking the damage.
The level of the trap is no factor, as it happens with traps with a chance of 115% to disarm and traps with 65%.
When it happens, he doesn't try and fail to disarm the trap, but simply activates it.
Which makes me believe that it is a pathing issue.
